please note , im sure from my all previous tests it is something related to musceles or nervous system not blood. so in case you prefer to consult with another doctors together it will be good idea since my problem is very complicated and it needs a doctor who has physiotherapy back ground to help me i think.

I do not think the erectile dysfunction is related in any way with kegel exercise and it is important to focus on psychological aspect of the problem.
Rather than wasting resources and energy on unimportant things please consult a psychiatrist in your city to move forward in direction of cure.
There is no muscle which is any way related to erection which can be corrected by physiotherapist. Physiotherapist are concerned with skeletal muscles although erection is more a smooth muscle thing. This is related to neurotransmitters which can be corrected with help of psychotherapy and medications.
