Question : I have a 25 year old son. Found a benign tumor on his pituitary gland about 6 months ago. High level of Prolactin, low testosterone. Started on Cabergoline and topical testosterone. Prolactin level is now WNL.
Hx of shinges to the right side of head about 2 years ago. Does have occasional severe headaches. Within the last 2 months he has had severe right hip pain. He can go days without the pain and then have a day or two with such severe pain that it brings him to his knees. Lyme's disease test is negative, CBC is WNL, non sensitve C reactive protein within range, Rheumatoid factor negative, antinuclear antibodies cascade within range, Automated ESR within range, hip xrays are negative. Uric acid is slightly elevated at 8.3. Can this level cause gout? know it is uncommon for it to present in the hip and the varied episodes of pain. Anything we should be looking for? Just don't know what else to look for. Thank you

                                    Brief Answer:
You should get him evaluated for SPA.
Detailed Answer:
Dear Madam,
Thank you for posting your query on HCM.
Well considering the age and history of hip pain two possibilities exist one is spondyloarthropathy or hip avascular necrosis . I would suggest to do MRI of hip joints to look for sacroillitis or evidence for hip AVN . If both these conditions are ruled out the probably its just a muscular condition.
I dont think this is gout. It doesn't present like this. In fact I will not consider it in my differential possibilities.
Take care.
