Do Variations In Menstrual Cycle Indicate PCOD?

Posted on Wed, 12 Oct 2016
Question: Hi

I am 26 year old unmarried girl. From the past 3 months my number of days of my periods have been reduced from 5 days to 2 days

Periods dates and days:
11 july (26th day) [3rd day didn't happen, 4th day brownish discharge]
8th august (29th day) [ 3rd and 4th day didn't happen; 5th day it did happen little]
6th september (30th day) (first two days good flow, 3rd and 4th day didn't happen)

So basically I used to get my periods for 5-6 days and now it has been reduced to three days, which is the cause of my worry.
I am unmarried and not involved in sexual activities.
I got all my tests done. Here are the tests results:
HB : 13.10
glucose fasting : 92
glucose(pp) : 79
insulin, fasting : 10.40
insulin, PP : 11
prolactin, serum : 8.95
TSH, ULTRASENSITIVE, serum : 0.999

Tests on Day 2 of my periods:
FSH : 7.09
LH : 5.59

Ultrasound of pelvic report has this written:
Both ovaries are normal in size/volume (Rt. 40X27X43mm:24cc, Lt: 28X30X38mm:16cc). Multiple immature follicles ranging from 3 to 6 mm are present mainly in periphery of both ovaries with increased stromal echogenecity.

My doctor says that level of my LH hormone is increased and I have polycystic ovaries which is cause of my worry. I am not even married and I have read that this disease causes infertility. My weight has been between 51-52 kg from the past 6 years. I didn't notice any such symptoms of hair growth, acne and obesity. Am I really suffering from PCOS and can it be treated by medicines.

My doctor has given mychiro tablet in prescription

Will this condition cause any hindrance for me to conceive in future. I am going to get married in 7 months and I am very worried. Please help

Brief Answer:
No PCOS

Detailed Answer:
Hi dear,
I have gone through your question and understand your concerns.
Only presence of polycystic ovaries in the ultrasound with no features like delayed irregular periods, acne, weight gain and hirsutism do not make the diagnosis of polycystic ovarian disease.
Only when other symptoms are present, then the diagnosis of PCOS is made.
Only polycystic ovaries are present in the ultrasound of many healthy females. There will be hence no problem in conceiving in future, as there is no PCOS. The LH value is normal in your case.
As long as periods are regular, there is no reason to worry for the flow.
The flow and duration of periods decreases in some females as the age progresses, and it is completely normal.
I will suggest you not to worry at all.
Hope you found the answer helpful.

Thank you doctor for your reply

I don't get it why my gynae is telling me that I have PCOS. She keeps on telling me that the LH should be 3.5

My doctor has given mychiro tablet in prescription to be taken for 3 months. And one more tablet to be taken from first date of the periods to 21 days and this is also for 3 months

I am not starting the medicine as I am very skeptical about having PCOS as I am not experiencing the symptoms I have read online. And moreover my wedding is approaching I don't want any other hormonal problems to arise after taking these medicines

The ultrasound report had this excerpt written

Both ovaries are normal in size/volume (Rt. 40X27X43mm:24cc, Lt: 28X30X38mm:16cc). Multiple immature follicles ranging from 3 to 6 mm are present mainly in periphery of both ovaries with increased stromal echogenecity :POLYCYSTIC OVARIES

Please guide me. Should I show my reports to some other gynae as well.

Brief Answer:
PCOS unlikely

Detailed Answer:
Hi dear, as I explained earlier, only presence of polycystic ovaries without other major symptoms is not at all PCOS.
LH values change according to the time of the menstrual cycle in which you had the test.
Moreover, there is no requirement of taking any hormonal medications.
I will suggest you to take opinion from other OBGYN, if feasible.

Brief Answer:
PCOD is PCOS

Detailed Answer:
Hi dear,
PCOD is polycystic ovarian disease and is another name for PCOS which is polycystic ovarian syndrome.
As I have explained earlier, you do not suffer from PCOD or PCOS.
PCOD is an hormonal disturbance and is not completely curable.Weight loss and lifestyle changes are the major treatment of PCOS.Diet includes restricted fat, sugars and plenty of fibres.
Hope you found the answer helpful.
