Brief Answer:
fentanyl and IDET suggested
Detailed Answer:
Hello,
You seem to have a quite large range of pain medications and still not successful.
You can use Fentanyl transdermal patches for pain.
You can consider microdiscectomy that is a surgical procedure that resects part of the disc to relieve pain.
Please discuss with your doctor for IDET (intradiscal electrothermal annuloplasty), a type of invasive surgery, alternative for back pain.
Hope I have answered the question.
Let me know if I can assist you further.
Thanks.

Brief Answer:
Lower dosage gradually.
Detailed Answer:
Hello,
Don't stop buprenorphine patches suddenly.
The dosage should gradually be tapered down every 7 days to avoid the potential for withdrawal symptoms. If you stop it suddenly you will experience withdrawal symptoms like dysphoria, restlessness, rhinorrhea, lacrimation, myalgia, arthralgia, nausea, vomiting, abdominal cramping, or diarrhea.
You can use low dosage packages every 7 days.
Talk to your doctor about it.
regards.
