Brief Answer:
you should take him if he has symptoms...

Detailed Answer:
Hello,

congestive heart failure is a very serious disorder and patients may get worse when their heart rate rises more than the can handle. You haven't mentioned his usual heart rate, the reason he's taking cetirizine and when he started it.

A rising heart rate may indicate various problems like arrhythmia (electrolyte disorders caused by the diuretics may contribute to them or even cause them), anemia, fever, thyroid dysfunction, etc. If this rise occurred after starting cetirizine then perhaps this is the cause, as cetirizine and other antihistamines may indeed cause a higher heart rate. Cetirizine may also cause fatigue.

So coming back to your question if this rising heart rate is causing him problems like shortness of breath or other serious symptoms then he should go to the ER immediately. Otherwise make sure you contact his doctor for further instructions.

I hope you find my comments helpful!
