Question : Will Azithromycin 500mg once for three days help in folliculitis reduction on back of scalp?

Yes, it would. You may take a tablet of Azithromycin once daily for 5 days rather than just for 3 days.
In addition I suggest you to use a topical 2% mupirocin ointment for local application.
