Question : Sir, I am a 25 year old man. Since, the last 6 years I have had very unhealthy food habits and lifestyle(But no alcohol). Just 3 months ago, I had piles(hereditary from my mother's side). Now, since 3 weeks I have had the symtoms such as nausea, dizziness, mental fatigue, feeling of having a cold or fever(although no temperature increase). One day, suddenly my hunger was no longer present nor did I feel the need to go to the loo. It continued for about a week. I checked the internet and saw that those were the symptoms of cirhosis. The doctor ( general + laparoscopic surgeon) whom I visited, could not find anything clinically. He prescribed me Udiliv 300 (ursodeoxycholic acid) (twice a day), Polybion( at night), Omez(twice a day before food) and Zofer MD (before food). After 2 days, I was feeling better and my hunger had returned. Hence, I stopped taking the medicines. I did a Liver Function Test and haemoglobin test.
The report of L.V.T. showed- bilirubin (2.8 mg/100ml), Conjugated(0.4mg/100ml), Unconjugated ( 2.4mg/100ml). All other ALP, SGOT, SGPT, albumin, globulin were well within limits. SGOT was just at the upper margin. Haemoglobin level normal.
I also had a USG and the result is as follows-
Mild hepatomegaly with normal shape. The parenchyma shows normal homogenous echotexture. Intraheptic biliary radicles not dilated. No focal parenchymal lesion is seen. Porta hepatis is normal. CBD= 3 mm and PV= 8 mm. No free fluid seen in abdomen. IVC/aorta is normal. Gall Bladder is partly contracted with increased wall thickness.
Just 4 days ago I had fever like feeling and paid the doctor a visit. The doctor saw the reports and asked me to keep taking udiliv300, polybion and Omez. He said that udiliv 300 will help in decreasing the bilirubin level. But what I saw in the internet is that Udiliv is used for dissolving gallstones. Am I getting the right treatment? Will the medication work? Do I have cirrhosis? Will the inflammation get normal? I still have the mental fatigue and the yellowish tinge on the skin has increased. However, bowel movements have become slightly better. And now the hunger is there and the feeling of my stomach fill is not there.

Hi
Thanks for writing in
I fell if you take rest avoid fried and fatty meals and continue with the medicines it will resolve
Udiliv is helpful in hepatitis as well
It does not seem to be cirrhosis, I would suggest you get the liver function repeated after two weeks
Hope this helps
Do write back in case of concern

Yesterday, I went to a gastroenterologist and he said that maybe I have Gilbert's syndrome. Why would hepatomegaly occur if it was just Gilbert's syndrome(which is harmless as he said)?

Yes Gilberts is harmless
Detailed Answer:
Hi
Thanks for writing in
As informed by your doctor Gilberts is apparently harmless situation where in the liver does not process bilirubin in a normal fashion
The reports show mild hepatomegaly which can occur in n number of benign liver situations and does not need active intervention if there are no symptoms related to acute liver disease
Just reassure yourself
Hope this helps
