default
Posted on Mon, 6 Apr 2015
Question: 45 y/o male, bulging disc in lower back and mid back. Has sciatica and nerve pain running down side of leg into knee. When erection occurs, has pain on right side of groin. Takes 2 baby Asa per day for history of blood clots. Denies painful urination, defecation.
I'm a nurse. Work for a family physician. But he won't go to see a doctor. And doesn't want me to ask her about it. I can feel the disc bulging.

Brief Answer:
Radicular pain/Please get a lumbosacral spine MRI done.

Detailed Answer:
Hi XXXX,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If a patient with this clinical history would have presented to me as an outpatient,I would have told that these symptoms most likely are resulting from disc bulges or disc herniation causing lumbo-sacral radicular compression. This probable diagnosis will be confirmed on getting a Magnetic resonance imaging(MRI) Lumbo-sacral spine done. Please get it done and be in follow-up.

Meanwhile, please start the following management regime-
1)Tab.pregabalin 75 mg once a day.
2)Tab.Duloxetine 30 mg once a day.
3)Tab.methylcobalamin 1500 mcg per day.
4)Tab.calcium and vitamin D supplementation.
5)Mckenzie spinal extension exercises-These should be learnt from an expert physiotherapist and doing these exercises regularly with proper technique is most crucial therapeutic step in the management.

Please follow-up,once the MRI is done.

Hope I have answered your query for now.If you have some follow-up questions,I shall be glad to answer else, please close the thread-rate it and write a review.Your rating will be of help to me.

He doesn't have health insurance and refuses to pay for an MRI. He has been using an inversion table which has helped a bit. The area of the disc bulge is swollen. And after erection which caused pain in groin, area of groin is swollen.

Please follow the treatment plan as advised.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up.

It would have been better if he could have got a MRI done.Anyway,Please follow the treatment plan as advised above.
Please refer to a general surgeon for the groin swelling. It must be physically examined by a surgeon. It might be infectious and unrelated to disc problems.
