Does Intake Of Norethisterone Help In Dealying The Menstruation?

Question: Hi there, I'm getting married end of July and my period would happen either on the day or the next 2 days after my ceremony. I suffer from really bad PMS (bloating to the point of looking pregnant non stop for 7 days before my period and 3 days after the start of my period, acne, tiredness, water retention, emotional, cramps, very big painful breasts which might no longer fit properly in my dress). I was thinking of taking Norethisterone to delay my next period due 4th of May, by 2 weeks, so that my period in July ends a week before my wedding day. But I've read for some (or all?) it only delays the period, not ovulation, so essentially I could expect me 2nd period 2 weeks after my next one (which would've been delayed by the drug). Is this correct? I've read you get the PMS symptoms for longer but I hope by "suffering" for 3 weeks in May, I avoid getting my period and PMS symptoms around my wedding day. Thank you

At the outset, let me congratulate you for getting married. I indded wish you a very happy married life!

However, you seem to be either misled or confused about the postponement of menses. Please get my advice on it to plan your menses properly.

There is no necessity of shifting the dates of menses from the month of May. It will no way help you to plan your menses in July.

If you are planning to postpone your pregnancy for at least few months after the marriage, this is the best time to start birth control pills from this month itself. Let your menses appear naturally fro the month of May. From second day of the menses, start birth control pills with the advice of your Gynaecologist. Till before the month of your marriage, take the pills regularly to have cyclical bleeding. Before the marriage, do not discontinue the active pills even after finishing them from pack, continue with the next pack till the time you are ready for the menses. After discontinuing the pills, you would get withdrawal bleeding within 4-7 days.

The other option is start the birth control pills 7 days before your expected menses around the date of marriage. Continue to have the pills till you are ready for the menses. After discontinuing the pills, you should get menses within 4-7 days. Accordingly you can plan to discontinue the pills.

By taking the birth control pills right after the menses of May will help you in many ways:

1. They will give you relief from PMS.
2. Your menses will be regularized.
3. You can plan your menses which are due around your marriage.
4. You will have contraception also.

From the history of your menses and the size of your breasts, I figure out that you are an Oestrogen dominant girl. So the ideal contraceptive pills for you would be Low Oestrogen / Progesterone dominant pills. Your Gynaecologist will advise you them in UK.

Dear Dr. Shrotri,

Thank you for your response. And sorry the name is Neece. I forgot to mention that I have a family history of breast cancer on my mother's side (3 aunts and maternal grandmother with breast cancer), and as a result the contraceptive pill was never prescribed to me due to the increased risk.

From your email you seem to say that taking Norethisterone to delay my next period in May would in no way affect my menses in July. Meaning after the 1st delayed period, my menses would resume as normal and occur on the same dates as if I hadn't taken Norethisterone. Is this correct? If yes, then I guess the only option is to take Norethisterone before my wedding?

Thanks for providing me your correct name. I am sorry to misspell it earlier.

You have almost read me correctly. Norethisterone taken in May will be of no use to plan your menstruation in July. I would advise you to start any progesterone preparation (Norethisterone is a progesterone preparation) at least 7 days prior to your expected menses and continue it till the date you ready to have menses any day. After withdrawal you will get the menses within 4-7 days. Since the exact lag period cannot be predicted, I always advise my patients to take it till the day of commitment and then discontinue; delay by few days is acceptable rather than getting caught in the awkward situation. Moreover, your menses are irregular. Hence, you might have to start the pills rather earlier than 7 days of expected menses.

The withdrawal bleeding after Progesterone preparations is always on lighter side. Hence, if your menses after these pills are scanty, please do not get worried about it. In fact Progesterone also may give you sort of protection against the breast cancer.

I entirely agree with you in avoiding Oestrogen preparations with the family history of Cancer breast. I would suggest you to give breast feeding to your child(ren) which also is supposed to give some protection against breast cancer.
