Question : Hi Doctor, I am 37 years old. I went to unprofessional massage centre today. There massage therapist was a female, who is doing massage for more than 5 years in different cities in Tamilnadu. My situation is she massaged me for 45 minutes and she did handjob for 5 minutes. No kissing and penetrative sex. I had a wound dried blood clot in leg finger, above the nails, which happen 2 days back. Assuming she is HIV positive and bleeding in her fingers and touched my leg fingers to massage. I saw her fingers and no visible bleeding in her fingers. Now I am worried and anxiety about my HIV transmission. I am married and have a kid.

Request you to answer my questions.

Does massage and hand job transmit HIV even if she is HIV positive?

Does HIV transmit through skin blood clot on my fingers? (Assuming she is HIV positive and bleeding in her fingers.)

Can I continue to have a sex with my wife?

Hi ,
Transmission of HIV through massage without any other sexual activities is almost nil. Hand job is the safest sexula activity. There is every chance for the parlor girl could be a HIV negative individual. Even if we consider that she is a HIV positive individual, when she is on regular ART drugs, and if her viral load is undetectable, she will be no longer infectious. Odd possibility is that she had a fresh wound in her fingers. Even if we consider such a possibilty then also the possibility of HIV transmission is negligible only.
I don't find any reason to abstain from sex with your wife if all the statement mentioned by you are true.

Thanks Dr. I will take test after 12 weeks. I am suffering from ED and here my sexologist prescribed L arginine XXXXXXX and tablet named Hemuria which contains ( L- arginine, hypericum perforatum, tribulus terrestris, withania somnifera, mucuna pritiens, ginko biloba, zinc sulphate momohydrate, and lycopene tablets) for 2 months. Also I am going for a jogging and gym for my fitness.

Request you to answer my below questions.

1. Does this medicine L- Arginine powder XXXXXXX and Hemuria tablet will affect and change HIV test result and it's window period?

2. Doing jogging and gym exercise will affect HIV test result and window period?

3. What type of medicine will affect HIV test results??

Immunosuppressive drugs can delay the seroconversion

1. This will not affect the HIV test report.
2. No, these will have no effect on the outcome of test report.
3. Steroids and any other immune suppressive drugs like cytotoxic drugs (in the treatment of chemotherapy of cancer patients) and Cyclosporine (given fortransplant recepients) and some other immune suprresive drugs and antiretroviral drugs taken as PEP can delay the seroconversion.

1.Why HIV testing specimen on serum mostly in India?

2. Which is best and more accuracy? Serum or plasma in terms of HIV?

Sanitizer has actions over the external surface of the skin over the body. It will not be absorbed through skin. Moreover even HIV virus, present over virus when exposed within seconds, also will be cleared by these sanitizers. Antibody production against virus is done by blood cells inside the body. Reaction by our immune system inside our body. Usage of the sanitizers will noway be affected the antibody formation and will not delay the window period of HIV infection.

Hi, Due to my negligence, the scissor which I used was accidently cut my friend's finger. Immediately, I apolgised him and sucked his fresh bleeding finger for 3 to 4 seconds. Immediately I took off his finger. Some of his blood came into touch my tongue. . I have some worry now., Does this transmit HIV? In case, if he is HIV positive am I at risk? Please help.

If we assume that your friend is HIV positive and you had come into contact with his fresh blood into your oral cavity, we cannot rule out the HIV risk totally.
1. If that friend is already on ART (Treatment for HIV) and if his viral load is undetecable, then the risk is almost nil
2. You have the risk of transmission of HIV, only if you have some ulcers in your mouth or some other Gum problems. Otherwise your risk is negligible.
When your friend is not HIV positive your HIV risk is almost 'NIL'.
You knew about your friend very well. Then why are you worrying?

You don't need any blood test for HIV with regards to this incidence and your massage parlor experience also does not warrant a HIV test.

HIV test 12 weeks after the exposure (4th generation COMBOtest 99% reliable and others 97% or more reliable).

Not at all. Boric acid application will not delay the window period.
Actually the window period for HIV is around 4 to 8 weeks only. 12 weeks for some rare delay in immune system of some individuals. Boric acid powder application is not causing any delay in the antibody production in HIV infection.
