Posted on Fri, 19 Sep 2014
Question: I am a 65 year old 5' 8" 170 lb woman. My latest lab work had a creative level do 1.35 and a GFR of 39. I have no issues with urine output and drink app 50oz of liquids a day plus I eat healthy fruits, veggies, fish ( little red meat). However I have taken 2400 mg of ibuprofen daily for 25 years for back and bone pain. When I Got the lab results I immediately stopped the ibuprofen and switched to 100 mg Celebrex twice a day. I have an appt with a renal clinic in late Oct. Will stopping the ibuprofen lower the creative level?what does having a level of 3.56 for ABC mean. Is it good or bad? what do levels of 107.3 for MCV and 35.7 for MCH mean?

Brief Answer:
Stopping Ibuprofen will reduce creatinine.

Detailed Answer:
Hello,

Welcome to healthcaremagic.com, I am Dr. Yogesh answering your query. I have gone through your question carefully and understand your concern.

A creatinine level of 1.35 (I presume it is mg/dl) could be considered highly significant particularly in elderly women, this along with a reduced GFR (39 ml/min/1.73m square indicates stage 3 kidney failure).

What these numbers mean is, you have damaged your kidneys permanently due to long term use of Ibuprofen.

Celecoxib (Celebrex) will not help much as it also has potential to damage the kidneys.

Stopping all medicines that can cause damage to kidneys will help in lowering the creatinine level but you will not get your kidneys back to complete normal.

By ABC I suppose you mean automated blood counts, and 3.56 is the counts of white blood cells, if that is so, then 3.56 is just under the normal range and is not a concern.

Value of 107.3 for MCV means you have larger than normal red blood cells, probably due to damaged kidneys. Kidneys are responsible for RBC formation through secreting a hormone called erythropoietin so since you have chronic kidney disease, it is likely that you will have anemia. MCH (mean corpuscular hemoglobin) is slightly above normal due to large sized RBCs.

You need to consult a nephrologist (kidney specialist) as early as possible and stop all NSAIDs (Non steroidal anti inflammatory drugs) including celebrex and switch over to other pain killers that do not have any damaging effects on kidneys.

I hope this answers your query. Please feel free to write back if you have any doubts.

Wishing you good health.

Warm regards.

Brief Answer:
Better to avoid all medicines. Opioids will help.

Detailed Answer:
Hello again,

Thanks for writing back.

In your case opioids will help relieve the pain and you can try tramadol but you will need a prescription to buy this medicine.

Hope this answers your question.

Warm regards.
