I read your query and I understand your concerns.
Following is my reply:
1) 90 percent head defects is a lot. But if remaining 10 percent sperms are having normal morphology then there is nothing to worry.
2) Conception is possible even if 4 % of sperms are having normal morphology. This report cannot be called terato spermia if more than 4 % sperms have normal morphology.
