Question: CARDIOLOGY
Dear Doctor,I am 75 year old male. I had IHD 40 years back (T wa ve inversion ). In 2011 i had hypothyroidism and in 2012, I had diabetes. Last year in May, before i left to usa i have had cardiology checkup. The ECG shown old T wave inversion which was there since 40 years in v4,v5 and v6 .Apart from that, the ECG is normal. My cardiologist wanted me to undergo 2 D echo and TMT also , since i have never gone to any cardiologist for the last 25 years. In fact this is the first time in 40 years to undergo 2d and TMT in my life .So underwent both of them.The 2 D echo is normal and TMT is negative. After my coming back from usa on 31st oct, on 2nd november i have felt a feeling of skipped heart beats .The beat missed one in 30 to 50 beats and some times one in 20 or even one in 5 o r 10 or 20.So i have not experienced chest pain or shortness of breath or sweating anytime . I went to my cardiologist .He checked my ECG and found nothing bad . I had some stress and strain while i was in USA. Since i am having Gas,Flatulence, and indigestion, he prescribed Rabium DSR and aristozyme. After 4 or 5 days, the skipping beats have gone and became normal. But occassionally, when i have stress and strain and gas, i am experiencing these sensations. I am experiencing particularly in the early morning. Again a week back i came from my 2nd visit to usa and my skipped heart beats are also come back . Here i should mention, i have taken Trika 0.25 mg for the last 7 months at some intervals. As i was in jet lag i have taken a tablet on 5th and 6 th of this month. Does this use of alprazolam cause ksipping heart beats?
I am taking Betacard (Earlier ATenlol since 40 years ,Ismo 10 mg, Amtas 5 mg and thyronorm 75 mcg.
My T3 is 0.9 ,T4 6.8 and TSH 4.8.Potassium 4.2

I have one doubt .When i told my cardiologist that i had skipped heart beats, he has written on the case sheet as palpitations. Are they both one and the same ?. Kindly examine my case in detail give me your valuable advise. Is this situation alarming? Do i have to do something or do i have to leave it just like that.Kindly advise

I have worked through your attached query in detail.

Skipped beats and palpitations are more or less the same.

Alprazolam can cause skipped beats in few individuals. It's advisable to limit it's usage.

Additionally Betacard can be protective against skipped beats.

Skipped beats have to be clearly evaluated.

Although many skipped beats are benign, few skipped beats can cause potential harm.

Single ECG is not reliable for skipped beats.

It's advisable to go for 24 hr continous ambulatory Holter ECG monitoring.

Normal 2D Echocardiogram and TMT negative are good prognostic factors.

Ultimately it's important to rule out arrythmias in all cases of skipped beats via Holter continuous ECG monitoring.

Further management if any is largely based on test results.

Review with reports to determine further course of action.

Please check with your cardiologist if he shares my view and if can prescribe this test to you.
