Question: Does this MRI picture of a nerve ANASTOMOSIS On my right arm of the AIN nerve to the Ulnar? As I had the surgey of the AIN end to side as I had a serious ulnar nerve entrapment that was released 5 month's along with the nerve transfer. I'm still yet to experience any benefit from this surgery as I still have muscle wasting and cannot spread my fingers and have clawing. When is it expected to have results from this surgery?

I have read your query and here is my advice.

There seems to be an anastomotic connection between the anterior interosseus nerve and ulnar nerve, but, the Radiologist's report and all MRI images are necessary for a more accurate answer.

There is a growing rate of approximately 1 mm/ in 24 hours after successful nerve repair and the follow up is done by sequential nerve conduction studies.

After three months there should be a progression of approximately 9 centimeters of the nerve repair measured from the exact site of the anastomosis.

It could take up to one year to see the full results of that type of surgery.

Yes Dr but I've not seen anything in 5 month's so by now I should of felt something from the anastomosis site where I had that done as it's suppose to have done something but I don't feel anything. Could my ulnar nerve just be dead.? And with the pictures I can't understand why the first is number 33 and second 34 but the second looks like a connection? Is that a deeper sequence?

It is necessary for the repaired nerve fibers to reach the muscles in order to see improvement.

The progression of the nerve growth is followed best by nerve conduction study.

So, in my opinion, you should get a nerve conduction study and EMG done in order to see if the anastomosed nerve is growing or not.

After that we can say if the nerve is dead or not.

The MRI numbering of the sequences is only a technical matter, it is not important the number of the slice.

MRI can give only anatomical information not functional one.

Yes Dr I was just wondering if that next number was a deeper sequence and hopefully that is the Anastomosis that's joined. I have Nerve conduction studies in 2 weeks time. Would a nerve biopsy or Diffusion tractography be of any use?

The numbering in the MRI slices is random.

Nerve biopsy is not an examination that is done routinely to follow up the nerve repair.

Tractography MRI may give some information about the nerve fiber's growth.

Nerve conduction study is the preferred examination to follow the progression of the nerve growth after surgical repair.

I will be having the nerve conduction studies in 2 weeks as required. Can Testosterone supplementation assist this process as I have heard that it's a neurosteriod and that neuro steriods such as Testosterone progesterone and estrogen can create better nerve growth?

Testosterone and androgen hormones, in general, are seen to have neuroprotective properties.

They also may promote axonal regeneration of motor neurons after injury.

However, you should discuss with your Doctor if testosterone is appropriate to be used in your case.
