Question: Dr. Kakkar,

I am doing better; however, I am still trying to figure out what works for what. I started a break out last week on the forehead, face and a little on the neck. Before the break out, I was taking the Benadryl @ 25MG but it did not prevent the reaction. I changed from the Benadryl to the Cetirizine which took the redness on my forehead, neck and face. However, I still have headaches, swollenness on my face and head with itching. Whenever, I have a little itching on my scalp and face, there is a knot on the back of my left ear. The knot usually go down as my skin heal but it has been there longer than normal. The knot has been there for over a month now.

Per the dermatologist I saw at the VA. I was scratching my head a lot and causing opening sores which could lead to infections. Even though, the Cetirizine is controlling most of the redness on my face and chest, I still have these symptoms on my scalp, itching, swelling, scaleness and a secretion of an oily and sticky liquid at the crown of my head.

What did you think of these symptoms for my head (scalp)? Do I need another round of Prednisone treatment?

Detailed Answer:
Hi.
I think your symptoms are because you seem to seborrheic dermatitis which usually presents as redness, greasy scaling and oozing on scratching.
It commonly affects face, scalp, chest and area behind ears .I suggest you to use a topical antifungal e.g either Ketoconazole lotion or cream. You may also use a ketoconazole based shampoo for your scalp.

Dr Kakkar,

you are right with the symptoms on face, scalp, chest and area behind ears. My dermatologist prescribed the ketoconazole lotion along with alternating Sebex to the scalp; however, it is a constant battle with the symptoms going away or subsiding.

Is there any internal medication for treating eczema and other dermatitis skin issues?

Hi.

Oral Isotretinoin can be tried for severe seborrheic dermatitis of face and scalp. Apart from that oral Itraconazole for 2 weeks is a worthy option.
