If you have mentioned the age of your wife, obstetric history and any history of usage of contraception more accurate answer might have been possible.
Anyhow usually menstrual cycles with the length of 21 to 35 days can be considered as normal only.
And also minor fluctuations in the menstrual cycle and flow can also be seen due to different causes, among them heavy flow with pain abdomen can be seen due to structural causes like fibroid uterus, adenomyosis, pelvic infection etc.
Other general causes of fluctuations in menstrual cycles are psychological factors like stress and strain, changes in diet, sleep, weight, etc., general conditions like anemia, thyroid hormone abnormalities, etc.
So, as your wife still having heavy flow with pain, better to consult your Gynecologist once. By clinical examination and ultrasound the possible cause of the problem can be identified.
If any specific problem noted on clinical examination or on ultrasound she may need treatment pointing towards that.
