Fetish Towards Buttocks. Doing Anal Masturbation Periodically. Advise?

Question: Hi Dr. XXXXXXX

I am a straight male, 24. I have fetish towards buttocks. I am struggling hard to get rid of it. Meanwhile, I am doing anal masturbation periodically. I got the fetish because of girl I was in relation with & after the break up I am doing it more often.

I get naked, sleep on my couch reverse wards. Take some pictures of my buttocks. I apply some vaseline on a finger or pen and perform anal masturbation. This is letting me down emotionally. I want to get rid of it. I don't know why I do this though I wish to have a sexual relation with only a woman.

Yesterday, I masturbated anally using a pen lubed with vaseline & I am having pain on the upperside of the both the butt cheeks. What might be the reason? I'm little worried about it.

I have gone through your question in detail and I can understand what you are going through.
I understand that you have two different issues right now.
1) Anal fetish and associated guilt
2) Pain over buttock cheek.

Regarding your first problem, from what you have stated it appears that you fetish is troubling you a lot and you are certainly distressed about your habit. It seems quite clear that you want to get rid of this habit. There are two possible ways of doing so and mostly you will have to use both if them. First is the use of psychotherapy. According to the Freudian principles the fetish develops if there is an obstruction in the normal process of sexual development and the same can be treated with psychodynamic psychotherapy. The same is done by trained psychotherapist and it may take a long time may be afew months or years. Another way is to deal with the feelings and thoughts associated with the fetish with cognitive behavioural therapy. That will make you feel a lot better and it involves 8-10 weekly 20 min session with a trained CBT therapist.
The second way is with the help of drugs. IN your case there is surely an underlying depression which needs to be treated. The depression can be treated with the help of drugs such as Escitalopram and the obsessions about the habit can be taken care of by fluvoxamine or fluoxetine.
Regarding your second probelm regarding your pain in the buttock cheeks, it suggests that there some trauma with your sexual act. Anus being highly vascular healing takes place fast and it should not be a worry. However if the pain persists for more than 7 days then it has to be locally examined. watch for fever or bleeding. if this appears then you will have to visit a doctor early.
