Question: For 3 years, I have intermittently (2-3 X a week), usually in the afternoon or evening) had the following Sxx:
Heat sensation, sweating lasting 1-1.5 hours, slight dizziness (but not really vertigo) , followed by intermittent vomiting lasting abt 30-60 min., followed by feeling very cold after vomiting stops. MRI and ENT eval were negative. A nap, even a brief one, will forestall the attack, if I catch it early enough. Coming off 10mg qd seemed to help, but now it's come back.

Besides the melatonin, have you taken other medications?
Has there been any change in your weight?
What were the ENT physician's remarks after s/he evaluated you?

Please note, that the autonomic nervous system (not under conscious control) regulates our heart rate, blood pressure, and body temperature.

Vomiting and dizziness can happen if there is a malfunction of the brain.

Should the thyroid gland not be functioning properly, a patient can experience hot or cold sensations.

Sir, you have many symptoms which may or may not be interrelated.
Perhaps a neurologist will be able to help you.

Thanks for your answer. As a physician myself (psychiatry, and my medical knowledge is woefully out of date), I know about the autonomic system.

It is true that I have a complex set of symptoms (poor balance, dizziness, hot, then cold sensations, and vomiting), but they seem clearly interrelated, as they all occur together for a limited time (1-2 hours). I was hoping that I was describing a recognizable, if unusual, symptom complex that shows up somewhere in the literature.

The ENT specialist did a vestibular system eval, cleaned my ears, and did a hearing test. All were normal (for my age).

The MRI showed no lesions and only normal changes related to age.

I am and have been on no medications.

My weight has been constant for quite some time.

Apart from the Sxx I am describing, I have been in good health for my age. No cardiovascular problems that I am aware of, and no neurological symptoms. I have not had a thyroid test, but it seems to me if that were an issue I wouldn't have these very brief and self-contained episodes I am describing. Plus my BP and heart rate have always been normal, though I haven't had them tested during an episode.

Welcome back. It seems that aural causes have been ruled out. Thanks for your through replies.
And you know more about panic attacks and anxiety than I do.
I am left with caffeine overdose, dehydration, and partial seizures.
In some of the institutions where I trained there was a referral service, primarily for inpatients, but for outpatients also. The physicians who ran the referral service evaluated patients who had already been seen by the family physicians or internists and a diagnosis could not be made.
Perhaps you might inquire at the University Medical School Hospitals in your area if that service is available.

Thanks. I don't think it's caffeine, though I do drink 1-2 cups of tea a day and have one coke.
Dehydration, though, is a possibility, since I basically hate drinking water, despite advice to do so.
Partial seizures is an intriguing idea. It's as though someone gave me a dose of ipecac. There is no particular nausea associated with it, though I do have a loss of appetite. The vomiting just happens. It's as though something triggered my vomiting center.
Hmmm.
Anyway, thanks for your input and advice.
