For Privacy Reasons (I Can't Have This Show Up On

My problem is I have been throwing up for about 12 hours now, about 15 times total. There is black bits which I'm concerned is blood, and I assume the other yellow and clear liquid is stomach linings.
I am very confident this was brought on by excess alcohol drinking during a period where I ate effectively no food over a 1.5 day span. I can't get any food or liquid down so I obviously need an IV, but again that course of action is not available right now.
Is there any solution besides that of a hospital visit? This happened once before, about 6 months ago, but that was over a 6 hour span and only vomited 5 times.
Thank you for your time and assistance, it is very much appreciated.
BLEEDING VARICES
Detailed Answer:
Hi....
Welcome to "Ask a Doctor"service.
I am Dr Ajeet XXXXXXX
I have gone through your query and here is my opinion.
As per your the history narrated by you ,it is very likely that your liver has been functioning poorly due to chronic alcohol intake.
This raises the pressure in portal vein thus causing dilatations and bleeding from esophageal varices.
This is a medical emergency and you should get medical assistance immidiately.
This needs a work up in the form of CT scan of splenic and portal veins and endoscopic examination (sometimes capsule endoscopy).
Bleeding varices are usually managed by endoscopic ligation, application of pressure by means of balloon temponad and intrahepatic shunting.
On the other hand blood volume is replaced by intravenous fluids, albumin and Packed cells to prevent shock.
Drugs are also used to reduce the blood flow into portal veins,to replenish vitamin k,promote coagulation and to prevent infection.
You should abstain from alcohol and visit your clinician immediately.
Hope that I have answered your query.
Stay healthy and stay blessed.
Regards.

Thank you for your response. The vomiting ceased three hours ago. I recognize the immediate need to abstain from alcohol completely and totally henceforth.
My follow up question is that due to the clandestine nature of this drinking, it would trigger significant concern and consternation within my day to day life professionally and personally.
Is there any life-threatening threats if I fail to approach a clinic immediately, and hold off for three days to ensure that there is no pain within my esophogous and liver. I understand my liver is functioning at an extremely low level and a legitimate diet and immediate abstinence from alcohol is is mandatory as this sort of lifestyle will kill me if I continue.
In essence, while I know that no doctor in the world would recommend this, and I'm not asking for condoning of this course of action, but is there anthing immenitely life threatening if I wait 72 hours to reduce the swelling of my liver.
As a follow up, are there any medications I can acquire from a pharmacy that would be recommended in the interim before I visit a clinic for a CAT scan and all relevant medication undertakings.
With Appreciation, XXXXXXX

Detailed Answer:
Hi...
Welcome back.
Dear XXXXXXX ..I might sound harsh but no doctor in the world can predict about the timing of your next bout of bloody vomiting.
It may happen with in next few minutes and may not in next 6 months.
It would be best for you if you visit an emergency room immediately.
Next bout of a large bloody vomiting might cause a decrease in blood pressure, shock, hypovolemic damage to vital organs and blood loss anemia.
For the time being,I would recommend you ...
1.Complete abstinence from alcohol.
2.Keep yourself well hydrated.
3.A rich fiber diet.
4.Bland diet.
5.No greasy,fatty and junk meals.
6.Avoid tea,coffee,carbonated drinks.
7.Avoid hot foods (might cause dilatation of vessels and re bleed).
8.De stress yourself.
9.A drug group called "Beta blockers" decrease pressure inside of the varices, which can reduce the risk of bleeding.
The two most commonly used beta blockers used for prevention of bleeding are propranolol and nadolol.
You can take Propranolol 40mg once a day under medical supervision.
I would reemphasize to immediately approach a gastroenterologist for endoscopy and possible banding.
Regards.
