Question : For the last 2 months I have this pressure on the roof of my mouth, nose around ears and behind eyes/forehead while talking. It makes talking weird. No one has noticed anything wrong with my speech. The pressure sometimes feels like stiffness inside the cheeks and also jaw pressure.

I also ambiting my tongue like 3-5 times a day and have extra saliva in my mouth.

I am not sure whats actually causing this. Should I go and see a specialist for the same?

Do you think this is TMJ?

Could this be some neurological issue (I googled for the same and found some articles which said could be neurological)?
This has led to massive anxiety and i am hyper focused on my mouth and speech.

My mouth muscles are super tensed for most part of the day. These come and go and not present through out the day but I have it every day.

No issues will swallowing or chewing but only while I talking I get this pressure.

I also get pressure behind my ears and around my ears and back of my nose.

Can TMJ cause facial tension and the weird speech or like issue while talking?

What about excess saliva and tongue biting?

Also, can TMJ be chronic like every day thing?

Brief Answer:
Yes, facial tension can occur due to TMJ

Detailed Answer:
Thank you for getting back.

Facial tension can occur due to TMJ. I presume speech is by far normal, as you said no one has noticed anything wrong with your speech.

TMJ can cause daily symptoms.

Should I see a neurologist just for my anxiety?

Also what could be causing excess saliva and tongue biting? I do have throat reflux and taking medicine for the same.

Speech is normal, it feels like mouth muscles are super tensed and that causes some difficulty while talking and makes my speech seem like there's a slur but again not actual slurring and then no one has noticed anything wrong. I have also asked people if they find anything different in my speech and they said all normal.

Brief Answer:
Muscle tension can affect the speech.

Detailed Answer:
Thank you for getting back.

Tension affecting the facial muscles can affect speech. Same can also cause tongue bite. Excess salivation could be related to medicines or related to anxiety. Salivation is not a sign of any significant disease.

You can consult a psychiatrist or neurologist for treatment of anxiety.

So this is not anything serious or even to worry about, right?

Which doctor should I see for TMJ and how is TMJ diagnosed if you can help me with this?

Is tongue biting and excess saliva a neurological issue ?

Rheumatologist for TMJ?

And Neurologist for like what? For anxiety or just routine check up and for anxiety? So this does not sound serious and I can wait till the corona virus thing is over right? Or should I try and see someone as soon as possible?

If your symptoms are mild, you could wait. However, if they are troublesome, you could consult a Neurologist sooner.

If symptoms are mild, you could wait for a few months.

Detailed Answer:
Tightness in throat and back of mouth could be related to muscle tension and anxiety.

Yes, if your symptoms are mild and not troubling you much, you could wait for 3-4 months before visiting a neurologist.

ALS is generally an uncommon disease (fortunately). I see 3-4 cases of ALS a month and most are above 50. None of my patients with ALS were below 30.

I have one more thing to ask why do feel like a tightness in my throat and back of the mouth leading up the nose while talking?

Also, I read if there’s anything neurological it’s better to wait for 3-4 months as if it goes away by that time then it’s nothing to worry about, am I right?

One more question I am worried about als, so how common is als in XXXXXXX and how many people do you know as patients my age with the disease in India?

Brief Answer:
Tightness could be due to anxiety.

Detailed Answer:
Tightness of throat and back of mouth could be related to anxiety. Sinus, GERD and TMJ does not cause them.

Clicking sound may not be present in all cases of TMJ. Most cases have headaches and pain/pressure over jaws, ear and face.

Bulbar ALS starts with dysphagia- difficulty in swallowing food and dysarthria- slurred speech. EMG is used for diagnosis or exclusion of ALS.

"Tightness in throat and back of mouth could be related to muscle tension and anxiety."

Tightness in throat and back of mouth are they neurological symptoms? Can Sinus/Gerd/TMJ and anxiety cause them?

I dont have the clicking sound or jaw pain/pressure which you usually associate with TMJ. But i do have headaches and pain/pressure around my ears/nose and cheeks.

Thats the reason I am worried like whats happening.

One last question, how does a bulbar emg usually start? Am I am exhibiting or anything close to a bulbar symptom?

My symptoms are mild and i can live with them but they are certainly affecting my mental state and causing lot of anxiety and this fear of MND does not help. I also have a feel of mucus in my throat (which comes and goes but have this thing daily for the last 3 months).

Brief Answer:
Your symptoms are not suggestive of ALS.

Detailed Answer:
1. For bulbar onset ALS, we usually do EMG of tongue and mentalis (chin muscle)

2. Muscle tension is related to anxiety and it is not a sign of ALS or any other neurological disease.

3. Yes, symptoms of ALS continue to get worse over time, they do not come and go.

You can upload the EMG report.

I had a en emg in XXXXXXX for some carpal tunnel and the doctor decided to do some other muscles since i was there he was like since you are here i will check some other muscles. He check 10 muscles in total including 1 paraspinal muscle all normal including a clean physical checkup by a neurologist. This was in January and all these problems started after the neurologist visit in January.

1. EMG of the tongue or bulbar muscles to rule out bulbar onset?
2.Is the muscle tension a neurologic symptom/sign?
3.Also, the symptoms in Als never relax or never come and go but rather stay or are constant and get worse, correct?

EMG is normal.

Detailed Answer:
EMG is normal and there were no features of ALS in the EMG then.

Tongue pictures also look normal. However, wasting/atrophy of tongue muscles is more appreciated on the surface (top portion) of tongue, rather than the sides.

The next review with a neurologist should be after about six months from your last visit.

I have uploaded my emg and 2 pics of my tongue (i want to check with you if they are presentation of a normal tongue or they look like atrophy to you)?

So, based on my symptoms, I would like to get a suggestion from you, should I wait for 2 more months (that would take my symptoms to 5 months from when all this started) and then go and see a neurologist? As I have already seen one or there is no need to see one and rather I should work on my anxiety?

I also had some swallowing issue probably due to throat reflux but thats improved quite a bit and very close to normal

It feels like muscle tension when I talk not so much while eating.

You can wait for two months

Detailed Answer:
Yes, you can wait for two months before seeing a neurologist.

Yes, it looks like a combination of TMJ and anxiety. There is no feature to suggest MND.

Yes, your symptoms are not suggestive of bulbar ALS.

EMG should be repeated after 6 months from previous EMG, if needed. There is no need to do early EMG.

So rather wait for at least 2 months before i see a neurologist for follow up or I should just concentrate on my anxiety?

Again so this looks like a combination of TMJ and anxiety and not even close to MND?

They did not do my bulbar muscles (emg), am I still clear of ALS?
Is there a case of doing emg like early?
