Question : For the past few weeks, I have pain (a burning sensation) when I ejaculate. The burning starts in my right testicle and radiates up through my penis briefly. It does not last, only during the ejaculation. I don't seem to have any other symptoms, not pain during urination, etc. I have a been taking Tamsulosin HCL 0.4 MG for the past year for urgency to urinate. I have a slightly enlarged prostrate; physical in November including prostate exam with nothing remarkable. I am generally in good health and am 57 years old.

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Your symptoms seem to be related to epididimitis.

It is an inflammation of the epididymus.

I suggest to do a urine test and a complete blood count to exclude the possibility of a urinary tract infection.

I I also recommend to do a prostate ultrasound to check the prostate condition.

Meanwhile, I suggest using painkillers such as Ibuprofen 400 mg three tablets a day.

I suggest applying cold packs to the scrotum and
avoiding lifting heavy objects.

If the infection is confirmed through tests you should take antibiotics.

I suggest Ciprofloxacin 500 mg twice a day for a week, which is a prescription medication.
