For The Past Two Months I Have A Small Amount

Question: For the past two months I have a small amount of clear discharge coming out after I urinate or squeeze my penis. I had painful urination about a month ago and took amoxicillin which made the pain go away. Now all I simply have is discharge sometimes leaking throughout the day. I took an AZO UTI test strip about a month ago and it came up positive for leukocytes but not nitrates. I took another one about a week ago and it came up positive once again. I only have frequent urination and discharge when I squeeze or urinate. Is this a bad UTI? Would an STD make leukocytes show up on a UTI test? This is stressing me out and I just need some answers. I had unprotected sex and oral sex about 5-6 months ago. I am a 20 year old male.

Sexually transmitted disease can cause leucocytes to come out in urinalysis, especially if the first urine stream is taken.
Presence of increased secretions with urinary frequency combined with leucocytes in urinary test but no other specific findings for urinary infection (nitrates) increases the chance of this being due to urethritis which is inflammation of the urethra and in most cases in young men is due to a sexually transmitted disease.

Because of your symptoms (secretions and frequent urination) and presence of leucocytes in urine and also due to the history of unprotected intercourse I would recommend you to have samples taken for chlamydia, gonorrhea and trichomonas.
Because of unprotected intercourse it is also recommended to have some routine blood tests to rule out HIV, siphylis and Hep B as these can be silent for a while and not cause much symptoms so that the patient seeks medical attention (these tests are done routinely every time there is a chance for sexually transmitted disease like in the case of unprotected intercourse).

I didn’t have any symptoms like this until about 2-3 months after the unprotected sex. I frequently masturbate with soap which is always getting by or in my urethra which causes burning for a few hours. When I initially had frequent urination I stopped masturbating completely took amoxicillin and all symptoms went away. I then masturbated with soap again on and off and I have had these symptoms ever since then. Could that be a cause to urethritis? When I take the UTI test it is never first thing in the morning it is usually late at night.

Hello,

It may be that the symptoms are due to the local reaction and irritation from soap but this would be a ruling out diagnosis, meaning this would be considered after other more important diagnosis have been ruled out. When i said first stream of the urine it means the first part of the urine not the middle or the last part. This is the one that would most likely come out with leucocytes in STD but it does not mean other parts of the urine do not show leucocytes in STD. Morning or night samples do not differ much.

Symptoms from STD can start up to a few months after the unprotected intercourse and in some cases a person can have an STD and not have much symptoms, most of the patients with chlamydia and gonorrhea do not have symptoms at all. So even just the episode of unprotected intercourse warrants check up for these infections.

So to conclude i would say:
- it is safer to have the tests and rule out STD first
- if these tests come out negative (normal) then the likely cause is the local irritation from soap.
