Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
I understand your concern.
Grade IV of varicocele is usually treated through surgery.
This treatment usually is done during general or local anesthetic.
The pain from this surgery is mild but might continue for several days or weeks.
So, after the surgery, you may need to take painkillers such as Ibuprofen.
Moderate exercise or walking is also recommended after recuperating from the surgery.
Hope I have answered your query. Let me know if I can assist you further.

Actually, I am interested in trying painkillers and routines to avoid surgery.
I am not interested in this time in treatment after surgery.
So actually my question not answered.

Brief Answer:
Follow up query explained below.
Detailed Answer:
Hi again dear,
As I explained above, this grade of varicocele is usually treated through surgery.
If you want to postpone surgery in another moment I recommend using over-the-counter painkillers, such as acetaminophen three tablets a day.
I suggest applying ice to the scrotum and groin.
I also recommend doing Kegel exercises in alleviating the symptoms of varicocele.
These are easy pelvic exercises that, when done consistently, can improve and restore muscle tone and blood flow.
You may check for this type of exercises to do them correctly.
I also suggest to use a jockstrap during exercise or prolonged standing.
Hope my answer was helpful.
