Question: Greetings. Several months ago I was found to have a granuloma in my throat. The ENT doctor said it was related to inflammation connected with acid even without serious symptoms. I was put on a proton pump inhibitor (imeprazole 40 mg x1 d) for several months and the granuloma disappeared. Now upon my follow-up a new granuloma has appeared on the opposite side of my throat, so the doctor is putting me back on the PPI after a 2 month interval without it. But it is unclear if this situation will just keep repeating over and over. The doctor also said that if I have no significant symptoms I don't have to take the PPI. My concern is: will this cycle have to continue repeatedly with granulomas, and even if I have a pause of the PPI for several months, will this have any negative side effects? Thank you.

Welcome.
Granuloma throat due to reflex without any serious symptoms are treated conservatively. Treatment option includes reducing the reflex by using PPIs. Along with ppi , you should also follow other measures to reduce reflux like in case of obesity - reduce weight, avoid alcohol, smoking, avoid spicy food, avoid coffee citrus fruits.
Avoid heavy meals, go for frequent small meals.
Go to bed atleast 45 minutes to 1 hour after meals.

PPIs for reflux are prescribed for longer period and they are relatively safe without much side effects. Apart from ppi , even inhaled steroid may be help, you can consult your doctor about it.

Thank you for your reply. My concern is why after many years of the reflux issues, only now has this granuloma issue arisen. Is this just going to go on permanently? Is it required to have pauses of a certain number of months in the use of the PPI drugs to avoid negative effects on body calcium or other effects of the PPI drugs? Why don't doctors advise repair of the hiatal hernia instead? Thank you.

Granuloma due to reflux are likely to resolve when reflux is controlled. Ppi in high doses given for very long period can affect calcium absorption but it is also not very common. Hence based on the information given by you side effects due to ppi are highly unlikely. Surgery is considered in case of severe symptoms and when all conservative approach fails. Decision can be better taken post examination. Hence I would have suggested you to follow the measures to takle reflux as mentioned above.
