Had An Echo Test And Taking Medication. What Do You Recommend?
Question: This is wrt to my Mother - XXXXXXX XXXX Age 68 yrs . attached are the details of her present medication and the summary of her ECHO done.

1. All investigations are best interpreted in light of clinical information. Their value is much reduced in the absence of clinical information which includes history and examination. With this rider, I will interpret these result.
2. Mild mitral regurgitation seems an isolated finding (no associated pathology of mitral valve or Left ventricle) on ECHO and is usually benign.
3. Her ESR is high. This may be raised in a number of conditions like anemia, inflammatory diseases, certain blood disorders and need a repetition and strong clinical correlation.
4. Her hemoglobin is slightly low, along with low MCV and low MCH. This is usually suggestive of iron deficiency. This may also occur in chronic inflammatory disorders.
5. Her neutrophil count is marginally raised. This abnormality may be trivial in nature as TLC is normal.
6. Her C-reactive protein is higher than normal. This may occur in infections which may be serious or trivial depending upon clinical scenario. Cardiovascular risk is as yet not fully understood with high CRP.
7. Her albumin is low. This may be due to improper protein intake in body.
8. Her alfa feto protein, rheumatoid factor, blood urea, serum creatinine, calcium, phophorus, alkaline phosphatase, uric acid, CA-125, CEA and CA 19.9 are within normal limits.
To sum up, She needs to consult an internist for her high ESR, Possible iron deficiency, low hemoglobin and low albumin.

1. Her overall ECHO report has been discussed in last answer and has no significant pathology. For cardiac status more important is her ability to exercise like using stairs. If she can use 1-2 flight of stairs at normal pace without any chest pain/ shortness of breath/ discomfort, her cardiac status should be reasonably fine. As evident from prescription she is diabetic, hypertensive and is also receiving treatment for diabetic neuropathy and anti-cholesterol drug too. Since ECHO report is near normal, much can not be said about her therapy. If she is maintaining good blood pressure control, good diabetes control and cholesterol levels with in normal limits (i.e. LDL cholesterol < 100 mg%) then therapy is good. Otherwise it will need modifications.
