Question : Hello Doc

Background
Male, 9 months
Birth weight : 3.5 kg
Current weight : 10kg
On mixed feed : expressed and formula ( aptamil)


History
- Ongoing cold since birth specifically sneezing and at times coughing
- Sweating over the forehead specially
- saturation of around 94% most of the times
- heavy breathing at times during the night
- has been diagnosed with UTI at the age of 2.5 months
- poor appetite since birth
- gastric issues and reflux issues till now
- pain and itching in the ears


Medications so far:
- Amicacin for 7 days during UTI
- nebulization with 7% saline and then normal saline has been doing on and off from 3 months
- syrup Sinarest for cold
- nasal spray - nasivion and otrivon P

He has been diagnosed with ASD ( Fossa Ovalis ASD of 6mm) and the size of the ASD has increased from 2mm to 6mm in 2 months time frame. does this require any further investigations like a 3D echo or any other time of ECHO ? and what is the reason for increase in size drastically

Hello,

After going through the medical query raised by you I can completely understand your concern for your baby‘s health and I would like to tell you that the symptoms which you are baby is experiencing like 94% saturation occasional breathing issues and poor appetite can be very well related to this ASD.

40% of the EST close spontaneously by 2 to 3 years of age. An increase in size of 4mm in just 2 months is bit unusual so it’s advisable that you should get a review echo done from some other good paediatric cardiac centre just to confirm and eliminate any operator dependent variations. 3D is a good option but requires hands of an experienced operator.

Kindly get a repeat echo done.
