Question: I have an anal fissure and I believe it is in part caused by dehydration . My first BM in the morning is difficult but as the day goes on I get better hydration. I have been seeing a colon rectal physician and many different creams also Sitz baths also 100 oz per day water. Is there a non traditional way to mosition the anal cavity in the moening before first BM . XXXXXXX XXXXXXX

Hi,
thanks for writing in.
Any cause leading to constipation and straining at stools can cause or aggravate a fissure in ano.
I would suggest the following if you are already suffering from it.
1. Plenty of water
2. plenty of fiber
3. stool softners till the lesion heals.
4. hot water antiseptic fomentations after passing stools and having cleaned the area with soap water.
5. local anaesthetic jelly application to the anal region just to prevent severe pain
after passing stools
6.avoid applying steroid containing creams in that region .Try keeping the area as dry and clean as possible.

I take a fiber laxative twice a day. Use nitroglycerin cream and analpram. Any other suggestions. XXXXXXX XXXXXXX

Hi,
thanks for the feedback.
Nitroglycerine will cause chemical sphincterotomy and will reduce the spasm allowing the fissure to heal. If your stools are soft and do not cause any pain or difficulty while passing then it is fine. You can continue with treatment. You can follow few of the tips which I have advised for speedy recovery so that you will be able to discontinue the nitroglycerine cream at the earliest.

All of what you had told me I am already aware of. I was hoping for a new suggestion, a magic bullet. If there are any other treatments other than Botox or an operation to increase the size of the anal passage please let me know. XXXXXXX XXXXXXX

Hi,
Besides surgery there is no method to increase the width of the anal canal to allow smooth passage of stools.
However this is indicated only in cases of severe spasm.
