Based on the history you have shared, it can be due to hernia or hydrocele but further evaluation by a surgeon can be helpful. Sometimes epididymo-orchitis may present like this so ultrasound testes and pelvis, as well as urine and semen analysis, may be needed.
Sometimes hydrocele may be present and spontaneously recover. Using tight underwear to support and avoid hanging loosely can be helpful to reduce swelling.
You can use OTC analgesics like acetaminophen or Ibuprofen which can help with inflammation as well as will give symptomatic relief.
Now, as per your description, the urologist may have found it to be benign which is why may not want to treat it but it's always safe to get it investigated and a second opinion may be sought afterward.

Yes it can be a possibility as hernia recurrence is an established complication however any other associated trauma to surrounding structures like lymphatic drainage can cause swelling.
