Cold hands and swelling in winters suggests connective tissue disorder.
The following are the possibilities.
1.Raynaud's syndrome.
In this condition the fingers become bluish, there occurs pain and swelling of fingers, on exposure to cold.
2.Scleroderma.
Here also there is pain tightening and swelling of fingers. There also occurs rashes on the fingers.
3.Lupus.
It manifests with pain swelling and ulceration of digits. The fingers may feel cool.
Here are my suggestions for you.
1.Always wear gloves.

You must get the following investigations done.
