There are three possibilities which should be ruled out as soon as possible.

1)     Testicular torsion: . Testicular torsion is characterized by excruciating one-sided testicular pain, with sudden swelling and is a medical emergency. You need to get a testicular ultrasound done to completely rule it out.

2)     Prostatitis: The typical symptoms of prostatitis are fever, pelvic, testicle, or rectal pain, frequent and painful urination and ejaculation and incomplete emptying of bladder.
During defecation, stool passing through the rectum may push on the distended prostate which may cause pain.

3)     Epididymitis: Epididymitis is swelling (inflammation) of the epididymis, the tube that connects the testicle with the vas deferens.

Depending on the diagnosis, treatment will be planned.
