Hello Doctor, Could You Please Let Me Know Which Drug

Question: Hello Doctor,
Could you please let me know which drug is more preferable to be used for gastric pain in the upper abdomen during 24 weeks of pregnancy. Actually, she tried pantoprazole 40mg in the morning empty stomach which relieved her pain for the day, likewise, she took it for few days which is relieving the pain. But whenever she is skipping the pantaprazole for one day, she is getting abdominal pain.

She underwent TIFFA scan which is normal. She is using folic acid, calcium, B complex and revital tablets now.

For your information she is taking 4-5 shorter meals in a day, avoiding spicy foods, drinking lot of water, but even then she is suffering with gastric pain.

1) Which drug(pantaprazole, esomeprazole, ranitidine, mucaine gel etc) would be better ?

2) Also suggest which vitamin and mineral supplements to be taken now ?
The supplements which she is taking is sufficient or it should be modified ?

3) She is also coughing for 2 days expelling little cough. what would you suggest for this symptom ?

I have understood your concern and I will suggest you the best possible treatment options.

First of all do not panic.

Basically, during the period of pregnancy, stomach emptying time gets delayed because of hormonal changes of pregnancy.
Secondly ,there is tendency of stomach contents to regurge upwards and cause symptoms of acidity and heartburn.

Please follow the 4 meal plan.
Non spicy food, avoiding green chillies is advisable.
I will suggest to avoid deep fried foods and bakery products and refined sugar in daily diet

As far as your questions are concerned, please go through the answers below.

1. Omeprazole, pantoprazole, Esmoprazole, mucain gel are quite safe in pregnancy.
However, indiscriminate use should be avoided.
2. All the current supplements are quite okay for her pregnancy.
Once she gets relief from stomach pain , she can have iron supplement, atleast half an hour before lunch. And she can now start taking protein powder supplement
3.For her cough and sputum , steam inhalation and warm saline water gargles can be started.

I would like to know about few things in detail.
1. When does she have her dinner, at what time?
2. Does she get vomiting alsi?
3. Is she following any exercise regime?
4. Is her cough associated with fever? and what is the colour of the sputum while coughing?

Ans: Dinner around 09:15 - 09:45 PM

2. Does she get vomiting also?
Ans: No, she didn't even have more vomiting in the first trimester also.

3. Is she following any exercise regime?
Ans: No exercise.

4. Is her cough associated with fever? and what is the colour of the sputum while coughing?
Ans: No remarkable fever observed externally, but she feels a little bit inside with chills, sleepy, feeling weak sometimes when she has chills. The colour of the sputum is white.

I have gone through your reply.

I'd suggest that she should opt for a healthy diet and regular exercise regime.
There are some exercises that are suitable for pregnant women. Also, regular walk for 20 to 30 minutes is also a good exercise to start with.

As you have conveyed her food intake patterns, it's good to have 4 to 5 small frequent meals.
She can seek prescription support from her treating doctor for Capsule Pantoprazole/Domperidone combination (eg.Pan D) at 8 am on daily basis for 10 days.

I'd also suggest warm saline water gargles and steam inhalation along with common cold tablets like Tablet Sinarest, as needed for her cough and cold.
