Good Morning,

Attached my annual health test taken at my home and there are some items marked as critical. (Report of 2018)

HDL Cholesterol
T. Cholesterol/HDL Ratio
Direct LDL Cholesterol
Bilirubin Total
Hemoglobin
Hematocrit
25 HYDROXY VITAMIN D-TOTAL

I can see that Liver is getting riskier year over year. Requesting your advise and prescription on all the alarming sections.

I have started to perform Tread Mill for nearly 2 months.
60 Minutes Continus running between 6.6 Km to 10.6, 12, 13 KM, 14 KM for 10 days
30 Minutes Continus running between 6.6 Km to 10.6, 12, 13 KM, 14 KM for 40 days

Reduced my weight from 82.5 to 77.5 KG. However i have started to get Knee pain / strain on my knees. Especially when i try to standup from floor or while climbing the stair case.

It has been nearly 40 Days since i have stopped Tread Milling, however the strain on my knees has reduced, however i can still feel the strain on my knees.

Could this to due to the Vitamin D deficiency.

I can certainly understand your concern...

I have worked through your attached query and reports in detail.

Your reports are suggestive of following abnormalities

1. Elevated blood cholesterol levels
2. Elevated hemoglobin

1. Elevated blood cholesterol levels can be frequently associated with hypothyroidism. This requires treatment with cholesterol-lowering agents like Statins.

2. Elevated hemoglobin levels in your case could be effects of smoking cigarettes/tobacco consumption. In case you're using these products kindly quit, and repeat the test after 1 month.

Likewise, an ultrasound abdomen will help to rule out any abdominal pathology.

Gastric symptoms require Upper Gastrointestinal endoscopy.

Knee pain could be due to osteoarthritis rather than vitamin D deficiency.

I forgot to mention that i am having severe constipation.

Hello Sir

I sincerely apologize for the delayed response owing to unexpected work schedules.

I have worked through your attached query in detail again.

Constipation is directly related to Hypothyroidism. Oral intake of dietary fiber like fruits, oatmeal, adequate hydration can combat constipation.
Avoid intake of cabbage, cauliflower, and broccoli.

Thyroxine is indicated only in cases of Hypothyroidism.

You are supposed to periodically get your thyroid profile checked to say once in 1 to 3 months to see if you are taking an adequate dosage of thyroxine.

You cannot go with a fixed dosage of 100 mcg for several years.

Dosage has to be adjusted in accordance with the latest TSH values.

Statins are a group of drugs to lower blood cholesterol levels.

I generally initiate a very low dose of Atorvastatin 5 to 10 mg to my patients with such lab abnormalities of elevated cholesterol levels.

For elevated hemoglobin levels, it’s advisable to get another test called PBS- peripheral blood smear. Further management is largely based on test reports.

You need to take up the test called Ultrasound scan of abdomen and pelvis.

Gastritis to be assessed by a procedure called “Upper Gastrointestinal Endoscopy”. This procedure is performed only by a Gastroenterologist.

Knee pain requires X-ray of both knees in standing position in 2 views “AP and Lateral Views. Consultation with Orthopedic surgeon always helps.
