Question : Hello Doctor,

I met with a car accident 7 days back, my car got hit by a truck from behind. I was sitting at the back seat of the car and was looking at the truck behind me when the truck hit. As a result, I got pain on the right side of the back. Doctor mentioned that the pain will go on its own. I waited for around 3 hours and pain slowly reduced. However, I started getting pain in the lower back (Right side) when I sit or drive for sometime. I went to an Orthopedic and he did an x-ray of the back (Spine etc). As per him, the spinal cord looks fine and there are no indications of Herniated disc. Also, he did other physical tests (For instance, Asked me to lay down and tried to move my leg in different directions) to make sure there is no nerve impingement.
After going through the xray report, doctor mentioned that it looks to be more of muscular pain as I was not sitting in a straight position (Was looking back) when the truck hit and should be fine in next 5-6 days.
He gave an Steroid medicine and a muscle relaxant for 6 days. As today is the 5th day, I am able to see improvement in the pain, though its not completely gone. Also, he asked to perform few exercises for lower back pain to strengthen the back/muscles.
Request for your advice if an X Ray is enough to come to the conclusion that there is no herniated disc and its more of a muscular pain. I am not having any pain moving to legs. Its more of a localized pain around the lower back (Area where we tie the belt).

Thanks & Regards,

Brief Answer:
MRI scan rules out herniated discs.

Detailed Answer:
Hello,
Thanks for posting your query.
From the symptoms that you have described this seems to be a muscular pain only. But to rule out the herniated discs, MRI scan is necessary. Since there is an improvement with medicines and exercise, you can continue with the treatment and get an MRI scan done to be very sure that there are no herniated discs.
I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.
Wishing you good health.

Hello Doctor,

Thanks for the advice.
I actually asked the doctor for MRI but he denied to order MRI saying that will be required only if the symptoms worsen and he is not allowed to go for MRI if it doesn’t look like a herniated disc as per the physical exam.
Request for your advice if I can go ahead with Gym exercises (Weight lifting etc) in such situation.

Thanks & Regards,

Brief Answer:
Avoid strenuous exercise for now.

Detailed Answer:
Hello.
Thanks for writing again.
You need to avoid the strenuous exercises till the pain subsides completely. The back muscles will take 2-3 weeks to tone up. After that you can start the exercises gradually.
Hope my answer is helpful.
Do accept my answer in case there are no further queries.

Hello Doctor,

Thanks for the advice. I just have one last question.
I stopped taking the Steroid medicine and Muscle relaxant since yesterday. I started feeling pain at below locations today :
1. Sometime in the right Hip area
2. Sometimes in the middle of Spine
3. Sometimes on the right shoulder
4. Sometimes around thigh area
5. Sometime around neck

I am not feeling pain consistently at a single location. Just wanted to confirm if there are any chances that this is a symptom of Herniated disc or this looks to be more of Muscular pain. Also, Just wanted to check if I can start taking Vitamin D supplement - 400 IU per day or is there any chance of it getting interacted with the steroid (Prednisone) or Muscle Relaxant (Cyclobenzaprine).

Thanks & Regards,

Brief Answer:
Muscle related pain likely.

Detailed Answer:
The pain at the different locations that you have is likely to be a muscular pain. This is not related to herniated disc. You can start with Vitamin D supplement but it is best if you get your levels checked and then start with supplements in proper doses. It does not interact with steroid or muscle relaxant.
Regards.
