Question : Hello, I am a 20 year old female and I have been having lower abdominal pain. A few night ago, the pain was so severe it woke me up out of sleep. It is not cramps it felt way worse. I went to an ER and they did blood work and urine test on me along with a ultrasound and they come out fine. But the pain is still there. It hurts more when I lie down. I feel it towards my left side and sometimes it shoot down to my groin. It also feels like it radiates. I also feel minor pain in my stomach so I don't know if it's a reproductive issue or stomach issue. My sister is a doctor and she said it might be Endometriosis but she wasn't sure. Im also getting lower back pain and migraines.

Welcome to Ask A Doctor service.

I have studied your problem carefully and I think I understand what is going on.

A pain that starts around the loin/small of the back and goes down in a spiral to the front of the lower abdomen towards labia and inner thighs is very classical of stones in the ureters.

This does not feel like stomach related or uterus/ovary related as the USG has come back normal.

There will usually be associated nausea and vomiting.

The pain does not let you rest, nor does it let you relax and do anything.

And it comes in waves.

This called ureteric or renal colic.

Antispasmodic medicines are better for this like dicyclomine rather than anti-inflammatory drugs.

Ultrasound scan does not pick up small stones and some stones are not visible to sound.

An X-Ray of the Abdomen can sometime show the stone or else you might need a CT scan or IVP.

Kindly consult a urologist and proceed further.

It is best to go back to the ER or to the hospital and get admitted, as the pain can become unbearable.

Were you due for your periods, in that case the bleeding is quite normal, not every period is similar, there will be some variation in the amount of menstrual flow and duration of flow.

If you were not sexually active or used protection, then there nothing alarming about the menstrual period.

This is not very likely to be related with the pain you were experiencing, how is that radiating pain bytheway? Are you feeling better, did you get yourself evaluated again for probable kidney stones?

I figured it was just my period, I was scared! But the pain isn’t as bad as before. I guess it was inflammation. But when I lay a specific way, I feel a slight pain and discomfort so I think you might be right about the stones. My brother had them last year and told me that’s how he was feeling so I will check them out soon. & it’s fine, it was the weekend I expected that!
