default
Posted on Mon, 15 Feb 2021
Question: Hello, I am still experiencing hypo symptoms. My recent lab values include TSH, Free T4, and Free T3. My T3 is past the reference range (4.98) while my T4 is at the bottom range (0.87). TSH is 3.317. This for my lab ranges. I'm currently on NP Thyroid 90 mg. What do you think of my numbers and should I add more T4 but lower my T3?

Brief Answer:
Repeat the thyroid tests/

Detailed Answer:
Hello
I have gone through your query and your attached reports.

Your NP Thyroid is a natural preparation derived from porcine thyroid glands that contain both the thyroid hormones or 38 mcg levothyroxine (T4) and 9 mcg liothyronine (T3) per grain of thyroid (or per 60 mg of the labeled amount of thyroid).

Adequate therapy usually results in normal TSH and T4 levels after 2 to 3 weeks of therapy.
Readjustment of thyroid hormone dosage should be made within the first four weeks of therapy,

Your TSH is normal but you have borderline/low fT4 and high fT3.Your numbers are not easy to interprete because i do not see the negative feedback mechanism that regulate the thyroid hormone activity.

In these cases,i suggest to my patients to repeat immediately the thyroid panel tests in another certified lab.

At the other side ,it is very important to evaluate the clinical findings.
You mention that you still have symptoms of hypothyroidism.

For this reason(if you can not repeat the thyroid test ) i will suggest to increase the dosis of your thyroid hormone replacement therapy and to repat the thyroid tests after 4 weeks.

Hope this is helpful.
Let me know if i can assist you further.
Wish you good health.
