Question : Hello, I first had itching and now pain on the underside of my penis, first below the head now on the head, it's now red and hurts. I believe it's thrush or balanitis.

The photo suggests either STD, herpes or thrush. It is paramount that you have a penile swab (or morning urine sample) and a swab from the vesicles to get a correct diagnosis. Is this possible for you?
