Hello and welcome to Ask A Doctor service.

I have reviewed your query and reports and here is my advice.

According to reports there is some high HbA1C, allergies, urinary tract infection (few bacteria) and crystals in urine. Are you diagnosed with diabetes? Do you get pain in back or joints? Any urinary symptoms or fever? Any allergies?
Amorphous crystals usually have no clinical significance. But we can still rule out any kidney stones and gout.

I will suggest fasting blood glucose levels, serum uric acid, urine culture and sensitivity and Ulltrasound kidney ureter bladder. These will confirm if there is diabetes, urine infection and kidney stones respectively.

Meanwhile take low sugar diet. Avoid high glycemic index foods. Drink lots of water (cleaner water from RO system).
High GI foods to be avoided: potato, banana, grapes, custard apple, sugar and foods that contain sugar, refined cereals and its products like white bread and corn bread, white rice, corn and its products. Also the regular milk flavoring powders like chocolate mixes etc. have added sugar and thus high GI values.
Controlling the sugar may control the urine infection also.
