I am Dr Sudhir Kumar, Neurologist, and I would try my best to help you.

I have noted your clinical details. I presume you underwent detailed evaluation to determine the causes of neuropathy and autonomic dysfunction. The causes can be in peripheral nervous system (PNS) or central nervous system (CNS). For PNS, nerve conduction studies are done for CNS, MRI brain is done initially. Subsequently, skin sympathetic response for PNS and DOPA PET for Parkinsonism may be done, if clinically indicated.

Bedside tests for autonomic dysfunction are done to decide if only nervous system is affected or other systems such as heart, bladder, gut, etc are affected or not.

If any cause is identified, that should be treated.

If no cause is identified, then, symptomatic treatment is given.

For neuropathy and allodynia, you could try duloxetine or gabapentin (as lyrica has failed).

For autonomic dysfunction, you could try fludrocortisone.
