Question : Hello!
I’m from XXXXXXX Indi.
On 1st december 2019, my dad got bitten on his hand by a dog who was very sick and paralysed. Dog died 3 days after the incident happened. My dad washed the wound and got vaccination immediately but being from a small village we don’t have very good faculty here. The first two anti rabies vaccination given to him was on buttocks but as soon as i got to know that buttocks are not the right sight to get injected, i went to another doctor and got rest of the vaccination done on the arm. Also the immunoglobulin (rabishield) was given to him on 7th day (7th december), not immediately. 3 weeks after the bite (21st December) his wound started to get pus. I’m so tensed why this is happening and what should i do. I’d be very grateful to you if you’d respond to my query. Thank you

Thanks for the query.
Arms are the preferred site for IDRV and nothing wrong in injecting at the buttocks.
In your whole course of vaccination is completed and immunoglobulins are also taken, and almost one month completed, so nothing to worry about rabies infections.
The wound infection is quite common with deep animal bite and that can be managed conservatively. Consult a general surgeon and get a thorough wound wash, a pus culture must be done from the wound and antibiotics must be started according to the culture reports. Most cases respond well to antibiotics.

Thank you. Also the immunoglobulin was just injected into the arm and not around the wound. It was written in the literature of immunoglobulin that it should be injected around the wound and remaining should be injected in the arm. So that’s okay too ?

Hi,
Ideally the immunoglobulin should be diluted and maximum dose to be injected around the wound to get best results.
However immune response will be there even if it is injected in the arm.
