Question : Hello, my name is Hati. I am a 20-year-old female. For the last 4 months, I have been dealing with an on and off fever that gradually is happening more and more often. It ranges from 99.8 to 101.4, and the last 2 weeks its been daily. I have night sweats, insomnia, extreme fatigue ( which I've been having for the last year), muscle aches, a very tender right cervical lymph node, dizziness, a strange aura that is affected by light, and a tingling numbness of my left side of the body. In the last 4 months, I've been taking antibiotics and pain relievers, but nothing seems to be helping. With this coronavirus outbreak and not having a Primary Care Physician, I was left with no other option than to seek it out here. My latest CBC diff I had high lymphocytes and low neutrophils, everything else was fine.I feel like I'm dying, I just want to know if the symptoms I've been experiencing are normal. If not what should my next steps be?

I am sorry to hear about your troubles. To me, what you are experiencing could be many things, e.g. mononucleosis, CMV infection or lymphoma/leukemia. I think it is paramount that you have blood tests and an assessment of the cervical lymph node, either by ultrasound or CT scan.

I am certain I don't have mono or CMV infection, do you think that anything else could be the cause? Also, do you think with these symptoms that I am having that it is mandatory that I should go get evaluated? I really do not want to go to the doctors and have them tell me that I'm fine. With the way that I'm feeling, I can't have them tell me it's normal.

Your symptoms are severe and diffuse enough to warrant further testing. If you do not experience worsening of your symptoms, you can wait it out. If not, you should definitely see a doctor.

Have you recently gone on any travels? Any type of narcotic abuse? Sexual intercourse? Known connective tissue diseases?

I have not traveled, I do not use drugs, and I am married (he's clean). I've had unexplained seizures in the past. But, if you say that I should get evaluated I will make an appt with a doctor and do blood tests. Do you recommend any type of blood test in case they don't?

I would recommend hematology parameters, I.e. hemoglobin, thrombocytes, leukocytes with differential count, M component and serology for CMV and Ebstein-Barr.
