Question : Hey doc,

Just a follow up since my last thread closed but basically I’ve had this burning pain when I pee on and off for about a year now. It mainly burns right at the start of urinating at the tip and I do feel a bit of pressure around my pelvic region. It’s a lot worse if I don’t drink enough water I find that’s the biggest problem or if I masterbate too much and I’ve always masterbated in the prone position growing up which I’m trying to change but I feel like that might possibly caused some minor damage? I also tried a fleshlight sex toy a couple of times last year and I feel like it kind of started after that a year ago,could I have maybe caught a low level bacteria infection, fungal or even parasite from that that never quite resolved? The problem is however I have had several urine cultures from different labs too and an ultrasound of my urinary tract since I first had it and they all come up negative for infection or inflammation every time. I also saw a specialist who discounted it as being anything and told me it was nothing bad and not to worry about it but it is very annoying to deal with that’s for sure. What do you think it could be and what do you think could of caused it? I have also never had sexual relations either so it’s not an std.

I also have irritable bowel so I’m not sure if that’s connected and maybe irriating the bladder also causing the pain? Is this something I will have to just put up with and deal with I just don’t seem to think doctors know what’s causing it. I have stopped masterbating for about 5 days now and haven’t noticed a huge improvement as of yet. Should I just be drinking a lot more water to help flush anything out?

One unsual thing I notice about my urine when it does burn is that a lot of white looking sediment comes out and just sits there and floats and it’s always when it burns it happens could it be dried up semen?

Brief Answer:
Could be either due to Urethritis or Prostatitis

Detailed Answer:

Thanks for query .

Based on the facts that you have posted burning urination that you are getting off and on since last one year is mostly due to one of the following two reasons.
1) Inflammation of urethra caused by trivial trauma during frequent masturbation that you have been indulged with
2) Infection of urethra due to contamination with cloths .
3) Infection of Prostate (Non Bacterial Prostatitis )

It is not uncommon not to detect presence of bacteria on urine examination or culture as it is the inflammation of urethra and not the bacterial infection .

Get following basic tests done to rule out prostatitis
1) Semen Culture
2) Microscopic examination of a discharge from urethra after prostate massage .

Taking antibiotic like Bactrim DS along with anti inflammatory drug like Diclofenac twice daily does help to reduce pain during urination .

Hey doc thanks for the follow up what I wanted to ask last time was if it were non bacterial prostatitis there isn’t a huge amount of treatment available for that right now and it’s hard to diagnose right? Those two tests you can’t do through your standard gp as well right I remember asking about the first one and they said that because the initial tests where clear they couldn’t do anything further id have to go back to a specialist.
