Question : Hey doctor,
Earlier before I went to pee and I hadn’t drunk much water all day maybe a cup or so and I had this painful burning feeling right at the tip of my penis right where it comes out not so much in the bladder or anything. What do you think it could be? Could there be damage where my urethra is that is sensitive when I have concentrated urine? I had the same problem a month or so back and went to the doctors to get tests and had no uti or infection what else could it be?

I have reviewed your query and here is my advice.
If infection didnt show up, then it could be non-bacterial inflammation of the area. It can have many causes like rash/dermatitis in the area, viral infection at the area etc. There may be some inflammation due to contact with cloth. Is there any visible redness or rash at the area?
You can try a thick cream like diaper rash cream or a steroidal cream like hydrocortisone cream and see if it reduces the burning.

I have reviewed your query and here is my advice.
If infection didnt show up, then it could be non-bacterial inflammation of the area. It can have many causes like rash/dermatitis in the area, viral infection at the area etc. There may be some inflammation due to contact with cloth. Is there any visible redness or rash at the area?

No redness or anything like that I feel as if the burning is inside the urthea not so much on the outside cream wouldn't really do much in that case right since its not on the outside? I feel that it is inflammed on the inside but it's been like that for a while now.

A complete urine examination including urine culture ad sensitivity be done, urethral swab can also be sent for microscopy. An ultrasound scan to rule out renal stones be taken. Kidney function test in blood work to find any kidney cause.
Drugs like pain-killers, antibiotics be avoided as they can also aggravate the situation.
Unfortunately, we cannot apply creams inside. Instead we can drink lots of clean water to flush out the inflammation. The condition doesnot seem bad enough to need oral medicines. Intra-urethral injection of steroidal preparation can be done by a surgeon or trained family physician in case of bad symptoms.

Yeah I had already done a complete urine examination for everything, a bladder ultrasound and some blood tests which were all clear I’m not sure if there would be any other ultrasound if another area that would be helpful for stones? I find if I drink water there’s no pain at all it’s just normal but when I don’t drink water that is when there’s issues and it stings bad with a bit of an odor in the urine but as soon as I start drinking a lot of water it fades away.

Could a scope just create further damage though if there were scar tissue or damage in the urethra would it not really be worth it? Do drinks like cranberry juice help and drinking more water?
