Hey Doctor, For About A Year And Half Maybe Close

Question: Hey doctor,

For about a year and half maybe close to 2 years now I have had this burning sensation when I pee. The burn comes on at the initial flow of the urine and becomes more intense as the urine flows through and then quickly disappears once the urine exits. It hasn’t gotten better or worse since I first got it and I got it before I even had sexual contact with anyone as well so this was pre that happening and that only happened for the first time this year so it’s not related to an STI however about 2 years ago I was using one of those fleshlight sex toys multiple times that I bought could it be possible I got a fungal or bacterial infection from that and it’s maybe hung around for this long or spread to my prostate? I’m just unsure what it is and I have had numerous urine samples and swabs done over the time period I’ve had it that have shown no infection or inflammation. I also visited a urologist about it 6 months ago and he didn’t seem too worried about it at all however it does concern me and I’m unsure what else to do or should I not worry about it? The only other thing is I currently have fungal infection of the penis glans which is being treated with an antifungal cream however this hasn’t made the pain better or worse. Can this be something more serious like cancer of the prostate/urethra I’m just unsure what to think?

Brief Answer:
Opt for a cystoscopy

Detailed Answer:
Hi XXXXXXX

I am sorry to hear about your troubles. A fungal infection can lead to the symptoms you describe. However, since this is a recurrent and chronic condition for you, I suggest you opt for a cystoscopy at a urologists, which is an endoscopy of your urethra and bladder in general anaesthesia.

Hope this helps. Please let me know if you have further questions.

Sounds like an infection

Detailed Answer:
Hi again XXXXXXX

As per the other response your symptoms suggest an infectious or irritative condition in your urinary system. A cystoscopy should be the next step.

Best wishes, let me know how you feel about that.

I’m not really super keen on a cystoscopy at all especially since my urine samples showed no inflammation or infection and I’m not having any blood when I pee. What do you suspect it could be based on what I’ve described? Is there any treatments I can try or less invasive/risky tests I can do that might get to the bottom of it? So far I’ve only done the urine samples, swabs and blood tests which are all normal. Do you think it would be something serious having it so long based on my symptoms not getting better or worse?

I also have found if I don’t drink a lot of water the urine is concentrated the burning is a lot worse but if I drink a lot of water it’s not really painful.

Hey doctor just a follow up I’ve noticed my penile skin on the right side is very, very thinly looking after using the steroid cream for now about 10 days. Can this be permanent damage do you think from the cream in a little worried. I can see all the veins on the right side I’m going to stop using the cream for now but it has me worried.

Brief Answer:
Try a steroid break for a week or two

Detailed Answer:
Hi again XXXXXXX

Long-term use of steroid lotions can lead to atrophy of the skin. Ten days is not that long, but it is fair enough to try a week or two without the steroids and see how it goes.

Best wishes.
