Hi Doc My Sisters Sugar Level From Past A Month

Question: Hi Doc

My sisters sugar level from past a month weeks has been spiked a lot.

To control with diet, she started intermittent fasting, by skipping breakfast from couple of weeks.

So, below is the full day report of yesterdays sugar check. Its done at home with Sugar Monitoring machine and its usually same or more on everyday basis.

7:00 AM - 196 - Empty Stomach
10:00 AM - 210 - Empty Stomach
1:00 PM - 217- Empty Stomach - Before lunch
3:00 PM -234 - After Lunch - 1.5 hours later food
9:00 PM - 214 - Before Dinner
11:00 PM - 242 - After Dinner - 1.5 hours later food

Please review the medication and prescribe to have better control.

Hi,

For better advise I would like to know following details about your sister -
- Height & weight.
- Family history of Diabetes or heart or paralytic disease.
- Family size completed or not?
- Any other Co-morbid conditions like blood pressure, thyroid or heart issues.
- Is she following diet for diabetes or not.

Considering details available I may suggest -
1) To start Tablet containing (Teneligliptin 20mg + Metformin 500mg) 1 Tablet with Breakfast.
2) Continue Tablet Metformin 500mg twice a day as before.
3) Keep following Diabetic Diet.
4) Daily 30 to 45 min moderate physical exercise as tolerated.
5) Emphasis on weight control - Ideal BMI around 23.
6) Keep monitoring Blood sugar levels at home.
7) Get following tests done after 1 month-
- Fasting & post prandial blood sugar levels.
- HbA1c
- Lipid Profile.
- Liver Function tests.
- Renal Function test.
- Ultra TSH.
- Urine R/M
If not done earlier.

Thank you for your reply. Below are the details.

- Height & weight. - 169, 63Kg
- Family history of Diabetes or heart or paralytic disease. - Mother and sister
both diabetic. Family history of Diabetics.
- Family size completed or not? - Yes. Husband and two kids. Boy and a girl.
- Any other Co-morbid conditions like blood pressure, thyroid or heart issues. - No
- Is she following diet for diabetes or not. - Intermittent fasting with little control on diet.
