Question: Hi Doctor,

My husband just got a general health checkup done and there are several deviations from normal. The particular ones i noticed are triglycerides, vitamin d and vitamin B12. Could you please refer the attached blood report and advise any medications or diet changes as necessary..

Hi,

I went through the lab reports of XXXXXXX uploaded here. There are few abnormalities and the notable ones are mentioned below:

1. Vitamin B12 is low. I would like to know if XXXXXXX eats non-vegetarian diet or if he is a pure vegetarian. Vitamin B12 deficiency presents with neurological symptoms apart from anemia. His blood hemoglobin is normal - so no anemia. If he has neurological symptoms such as burning sensation in limbs or weakness, then it needs to be evaluated.
Vitamin b12 deficiency can be corrected with supplements. However an intrinsic factor test (level / antibody) is necessary to decide the mode of treatment (whether oral supplements or injection). He can contact a physician and discuss about the test.

2. Vitamin D deficiency. This deficiency can be corrected with weakly supplements of vitamin D (D rise capsule 60000 IU) given for 6- 8 weeks. His physician will know about it.

3. Triglyceride and LDL values are marginally high; but I don't consider it significant and concerning now.

4. Bilirubin values are slightly deviated - both total bilirubin as well as indirect bilirubin is high. This change is probably due to deficiency of an enzyme that metabolizes bilirubin. Such a kind of deficiency is seen in XXXXXXX Syndrome. XXXXXXX Syndrome is not a serious problem. You need not be overly concerned about it.

5. Marginally high red blood count, PCV and low basophil are not concerning issue. It doesn't need medical attention.

thank you doctor. he is a vegetarian. yes he continously feels weakness in calf muscle. can you suggest tests which needs to be done for vitamin b 12?

1. So are you suggesting to get a blood test - i.e. intrinsic factor test (level / antibody) done? Can we consult you after performing this test?
Are the blood reports sufficient such that your consultation is sufficient or is a physical/in-person analysis necessary?'

2. I'm not planning to consult another physician unless you suggest it's necessary. Is it sufficient to start D rise capsule 60000 IU supplement for now?

Apologies for the late reply.

The treatment suggested is to correct the report abnormalities; however the specific treatment needed is decided through careful clinical and physical assessment. Therefore yes, it is better to see a physician in person.

Nevertheless, the intrinsic factor is necessary. It will help us decide vitamin b12 deficiency treatment. And D rise capsule as suggested will correct the current vitamin D deficiency. The future treatment can be decided by the treating doctor.
