Question: Hi Doctor, Please let me know if I am suffering from Auto immune disease by looking at attached report. Currently I am suffering from Urticaria for last 50 days. Prior to that I had finger stiffness for 2.5.years and no other joints involved and had taken ayurveda medicine for 1 year on and off. Now suddenly got Urticaria and then Hepes zoster. I took medicine for Herpes zoster and feeling fine after a week. But Urticaria not going away. So, started looking from auto immune perspective. Appreciate for right guidance on this. Am I auto immune patient?.. What should be my next step to cure Urticaria and if at all autoimmune. Btw, RA factor negative, CRP normal, Anti ccp normal, ESR increased recently only 17. otherwise always normal.

Hello and welcome to ask a doctor service. I have gone through your query and noted your concern.

Urticaria can have multiple causes like certain foods, food preservatives, infections, medications etc. Many cases are idiopathic (90%)i.e without any discernable csuse. Although the mechanism of urticaria is immune mediated release of histamine from mast cells but it is not a specific symptom of any autoimmune disease. Therefore urticaria is not a basis for diagnosis of autoimmune disease. For urticaria i would suggest symptomatic treatment with oral antihistamines apart from basic tests like CBC, LFT/ KFT. Examination to rule out any infection like dental caries, chest X ray, urine routine and culture, stool for ova / cyst may suggest a cause of urticaria.

Hi.
Your reports are fine except that you may have a positive RH factor therefore it need to be repeated after a few weeks Raised ESR could be due to ongoing joint inflammation or recent herpes zoster. History of stiffness in finger needs to be carefully followed up with a rheumatologist as it could be due to a finger joint involvement specially in light of equivocal RH factor.
