Hi Doctor, This Query Is For My Daughter (Sanika Nair)

Question : Hi Doctor,

This query is for my daughter (Sanika Nair) 7 years old.
Height : 129 Cms
Weight 34 Kg
She is having itching over external genitalia for past 15 days. She also has pain while passing urine sometimes. But mainly itching over external genitalia most of the times.

We have done urine routine and culture test on 6th Aug. It shows Escherichia coli. (Report Attached)

We had shown the report to her pediatrician on 8th August. He had prescribed Tab ALTACEF 250 for 7 days (2 times a day) and Syp CITRALKA (5 ml 3times for 3 days). (Prescription Attached). Completed the prescribed course of medication.

But itching still exist.

Please suggest.

Thanks for query .

Based on the facts and Urine reports that you have uploaded your daughter seems to have UTI due to E -Coli .

As per culture report Ciprofloxacin(MIC being less than 0.25 compared to Cefuraxime being 4 ) is more effective than Altacef hence I would suggest you to stop Altacef and start Ciprofloxacin 250 mg twice daily .Normally one has to take antibiotics for 2-3 weeks

Female children at this age usually have secondary fungal infection which causes itching around genital organs hence I would suggest you to apply topical anti fungal cream or Gel Miconazole ( Zole or Daktarin cream ) over genital parts.

If possible you can get microscopic examination of scraping or secretion around genital to confirm the same .

Ensure to wash her genital area twice daily with warm water twice daily before applying anti fungal cream or gel .

Ensure to wash her inner wares in boiling hot water mixed with anti septic solution Dettol.
