Brief Answer:
A number of causes

Detailed Answer:
Dear Ma’am, I have gone through your question and I understand your concerns but this could be a rare side effect of mertazapine to cause arthritis, however this occurs in less than 1% of patients using it.. This can be excluded by physical examination by your doctor together with an X-ray.. If arthritis was excluded , then this could be due to neuritis due to vitamin b12 difficiency or a nerve compression which can be diagnosed by a nerve conduction velocity study..
if the overlying skin is red or tender or swollen then this might raise the suspension of an inflammatory condition or a dvt, although rare at this site..
This also could occur due to a muscle sprain , which can be resolved by an anti- inflammatory gel as voltaren gel every eight hours together warm compresses.. I hope you find this helpful.
