Question: Hi. I am a 61 year old male in good overallhealth but having difficulty controlling my blood pressure. I am currently on: 4mg/day Mavic (an ACE inhibitor), 2.5 mg/day indapamide (a diuretic) and 7.5 mg/day amlodipine (a vascular calcium channel blocker). My BP is consistently 150/84. When I started the amlodipine 8 months ago at 2.5 mg/day I had a dramatic affect and BP was 130/80, but the effect was short lived and increasing to 5mg and then 7.5 has not made any difference. I am not overweight. I exercise regularly.
Can you suggest why the amlodipine has stopped working? thank you.

After going through the medical query raised by you i can understand your concern and would like to tell you that although your blood pressure is slightly over the desired range for your age group and its good to know that you are maintaining healthy lifestyle routine.

Along with this i hope you are also keeping your dietary salt intake under check, its a very important and easily reversible factor for raised blood pressure.

Now coming to your question, some people develop resistance over time to amlodipine and some develop ankle edema as a side effect of it. I would like to suggest you to talk to your doctor and switch to cilnidipine for better results.

Also no major studies have been conducted that proves the benefits of using combination therapy with CCB’s and there are no recommendations in the hypertension management guidelines.

Hi again. It appears that cilnipidine is not available in Canada (or at least the province of XXXXXXX where I live). Are there other vascular CCBs that would be worth trying? The most common one here other than amlodopine is Felodopine.
Would that be worth a trial?

Or, what do you think of a more powerful diuretic? I sense reluctance on the part of my family physician to use beta blockers and I am sure there is a good reason for that. Thank you for your time. XXXXXXX
