Question : Hi, I am female, 73 years old. I have three children. I have been ill for several years with diabetes and severe back pain. During the last year I have lost 40 lbs without dieting. I have constant nausea and bloating which transfers to my back pain. I have also had major bouts of severe diarrhea. They found chronic appendicitis and removed it four weeks ago. Then they found gastrparesis. I am still in very major back pain still with extreme nausea and bloating. I get the sweats on a daily basis also. My anxiety rate is very high. What else should I be looking for. I am getting weaker by the day, I am only able to take in about 600 calories a day at the most. I just started taking R England 4 days ago, it's not helping and I'm in pain 24/7. I getting desperate . Can't sleep, can't eat, can barely walk. Could it be chloecystotis, what other tests should I ask for to find out what else is wrong. I'm not winning this battle, I need help

First of all my dear patient just calm down for a second take a deep breath and have faith in yourself you would definitely win the battle.
The first and the foremost thing you need to control is your anxiety. Nausea and bloating is very likely a result of gastritis and indigestion(as a result of gastroparesis) and anxiety worsens it many folds.When a person is anxious the autonomous nerves of the patient are stimulated which causes secretion of large amount of acid which irritates the soft surface of stomach causing nausea.
Initially I would suggest you to add a prescription drug-
Lorazepam and Paroxetine in your schedule(doses as decided by nerologist/psychiatrist).They would calm down your nerves which is very important.

loss of appetite loose motions and weight loss are all a result of Diabetic gastroparesis.Nerves are weakend and hence gastric and intestinal motility decreases.Therefore food starts stagnating in intestines causing production of large amount of gas causing bloating.This gas too exerts pressure on intestines causing nausea.
Add a probiotic like Econorm XXXXXXX thrice daily.
Take activated charcoal tablets twice daily to absorb gas.
Both are over the counter.
Now the back pain may be a result of either low calcium and vitamin D levels or can be nuerogenic in nature.Some times the vertebrae starts pressing the nerves hence causing pain.For proper treatment you need to get a MRI screening of backbone done and medicines would be started as per your pathology.
Till then you can use patches of analgesic.Use ointments containing diclofenac and can also take tramadol 50 mg tablets on precription.

Keep blood sugar in strict control.Meet a endocrinologist possibly he may put you on insulins for few weeks to make you blood sugars and HBA1C levels normal.
